Drilling out of the promontory for dehiscent VIIth nerve in primary otosclerosis
(Language: english, french). Left primary surgery for otosclerotic stapes ankylosis. Transcanal approach. There is a really narrow oval window with a dehiscent facial nerve. The oval windows remains really narrow even after stapes superstructure removal. The promontory is drilled out allowing for safe exposure of the stapes footplate for stapedotomy and vein graft interposition. Ossiculoplasty is performed with a 0. 4mm diameter Causse Teflon piston.
