Audience: Medical Interns and Residents: Surgery, Emergency Medicine, Radiology Learning Objectives: Differentiate pseudoaneurysm from active extravasation Recommend the next step in management based on key imaging features Summary: Pseudoaneurysm Follows arterial blood pool Does not change shape Active Extravasation Remains dense despite some degree of dilution Expands or pools Is the patient hemodynamically stable after resuscitation Attempt to manage nonoperatively CT with arterial, portal venous, and delayed phase Is there pseudoanuerysm or active extravasation Treat vascular injuries with angiography Higher grade injuries dont always need surgery, but Greater chance of delayed complications Consider Followup imaging in 48 to 72 hours
