I lucked out with an arming sword and some fancy boots. You mostly just crouch, spin around them, block the first double attack and swing once, or if they do the big swing you can stay crouch and get 2 attacks in after. The big thing is the ground slam. The trick is to slightly backpedal as you spin around so your are right up next to the cave troll. If you are clipping into him and glitching, bobbing upwards then you are too close. Another trick is to keep an eye on your sprint timer, once it gets to 510s left, they are prolly gunna slam the ground soon.
